INCIDENT REPORTING POLICY
This document sets forth the policy of the Granite Bay Youth Lacrosse (“Club”) for the reporting of certain incidents as indicated below:
I. Incidents Covered By This Policy
The following incidents are covered by this policy:
A. Any injury sustained to a player during any practice, clinic, or game in which the Club is participating. This includes any injury to a player on a Club team or any player on another team if (a) that player was playing against a Club team or (b) the Club was hosting the tournament or game in which that player was injured. It does not include an injury to a player on another team if the Club was not hosting the tournament or game in which the player was injured and was not playing the other team.
B. Any injury sustained to a coach, parent or spectator during any practice, clinic, or game in which the Club is participating. This includes any injury to a coach of a club team or parent of a player on a Club team or any coach, on another team if (a) that player was playing against a Club team or (b) the Club was hosting the tournament or game in which that coach, parent or spectator was injured. It does not include an injury to a coach, parent or spectator for another team if the Club was not hosting the tournament or game in which the coach, parent or spectator was injured.
C. Any ejection or removal of a player, coach or parent of a Club team from any game, clinic or other event in which the Club was participating.
D. Any ejection or removal a player, coach, parent, spectator from any game, clinic or other event that the Club was hosting or sponsoring.
E. Any other incident involving a player, coach or parent of a Club team which constitutes a violation of the Club’s code of conduct or which would otherwise detrimentally impact the reputation of the Club.
F. Any complaint received from a referee, coach, parent or player of another team concerning a coach, player, or parent of a Club team or concerning an event hosted or sponsored by the Club.
G. Any complaint or disciplinary action concerning a coach, player, parent of a Club team or concerning an event hosted or sponsored by the Club that is taken or proposed to be taken by the Northern California Junior Lacrosse Association.
H. Any complaint received from Granite Bay High School, Roseville Joint Union High School District, the Eureka Union School District or any other owner or operator of a facility being used by a Club team or being used by the Club for practice, games, or any other event.
II. Reporting Procedure
A. All incidents shall be reported in writing to the Club President with a copy to the Club Vice President. The report shall state the date of the incident, the names of the persons involved, a detailed description of the incident, whether there are any witnesses or videos of the incident, and what actions were taken in response to the incident. The report shall be provided within 48 hours of the incident, if possible. Reports shall be emailed or mailed to the Club President and Vice President.
B. The head coach of each Club team as well as the designated Team Parent of each Club team shall be responsible for ensuring that incidents involving their team, players or parents are reported under this policy. In cases where the incident does not directly involve a Club team, but occurs at an event hosted or sponsored by the Club, the Club shall designate a person for that event who is responsible for reporting any incidents.
III. Follow-up Action
A. The Club President and Vice President shall confer on whether any follow-up action needs to be taken in regards to a reported incident. If such action is required, the Club President shall convene a special meeting of the Board of Directors to determine the appropriate follow-up action or response in its sole discretion.
